Responsibilities

• Evaluates member needs, barriers to care, resources, and facilitates best outcome.
• Develops or contributes to personalized care/service plans with providers.
• Provides psychosocial and resource support to members and caregivers.
• Coordinates between member/family and care team to ensure timely access.
• Monitors progress toward goals and revises care plans as needed.
• Maintains member information to comply with regulators.
• Performs on-site visits to assess needs and collaborate with providers.
• Educates care managers and families on procedures and benefits.
• Other duties as assigned by people leader.
