Responsibilities

• Develops, assesses, and coordinates care management activities for members to provide quality, cost-effective outcomes.
• Develops or contributes to personalized care/service plans and educates members and families on services and benefits.
• Identifies providers and community resources and facilitates access to care in a timely manner.
• Documents and maintains member information to ensure regulatory compliance.
• May conduct on-site visits and provide education on procedures and benefits.
