Qualifications

• Experience in care management and discharge planning is preferred.
• Strong communication with patients, families and the care team.
• Knowledge of post-acute care resources and community services.
• Proficiency with EMR systems and standard workflows.

Responsibilities

• Provides grief counseling, disease adjustment support, crisis intervention and goals of care planning.
• Assesses discharge planning needs for inpatients, observation and ED patients.
• Reviews medical records to inform transition of care plans.
• Develops discharge plans and coordinates post-acute care arrangements.
• Uses technology to communicate with post-acute services and ensure complete records.
• Participates in multi-disciplinary rounds to align resources and timelines.
• Educates patients and families about illness, impacts, and community resources.
• Leads patient/family care conferences with the team.
• Documents evaluations, plans, barriers, and discharge progress per standard work.
• Advocates for patient rights and coordinates with payors as needed.
• Assesses readmissions to identify reasons and prevent recurrence.
• Performs other duties as assigned.
