• Assess discharge planning needs and coordinate timely transitions across inpatient, observation, and ED settings.
• Review medical records and coordinate post-acute care with social and financial considerations.
• Organize care conferences, document plans, and facilitate multidisciplinary rounds.
• Communicate with payors for post-acute authorization as needed.
