Job summary

AdventHealth in Ocala, FL is seeking a skilled Care Management professional to support discharge planning and post-acute transition coordination. The role focuses on holistic patient assessment, family guidance, and timely care transitions to optimize outcomes.

Qualifications

• Master's Degree in Social Work required.
• Field of Study: Social Work required.
• 2+ care management experience in acute care setting preferred.
• 2+ social work experience required.
• ACM or CCM certifications preferred.

Responsibilities

• Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate.
• Assesses patients’ and families’ discharge planning needs across inpatient, observation and ED to avoid delays.
• Reviews medical records and integrates clinical, social, and financial factors into discharge plans.
• Develops discharge plans with contingency plans for timely care coordination.
• Communicates with post-acute care services to ensure continuity of care and accurate discharge reconciliation.
• Leads multi-disciplinary rounds to review patient status and plan for timely transition.
