Qualifications

• Bachelor's degree required; Master's preferred.
• 1+ year of direct care management experience.
• 2+ years of social work experience.
• Knowledge of CMS Conditions of Participation for discharge planning.
• Ability to lead others while ensuring compliance.
• Strong communication and data analysis skills.
• Experience generating reports and using PC applications.

Responsibilities

• Oversees discharge planning and social services in inpatient, observation, and ED.
• Implements corporate care management strategies with facility leadership.
• Participates in rounds to ensure effective discharge planning and barrier resolutions.
• Provides care management expertise to the team.
• Monitors documentation quality and regulatory compliance via reports.
• Facilitates care conferences for complex transitions.
• Participates in meetings to review high-cost or long-stay patients.
• Addresses complaints and escalates as needed.
• Involves in hiring, onboarding, mentoring staff.
• Performs other duties as assigned.
