Job summary

AdventHealth in Florida seeks a skilled Care Manager to support discharge planning, transitions, and post-acute care coordination for inpatient populations. The role requires a Master’s degree and 2+ social work experience, with ACM/ CCM preferred.

You will collaborate with a multidisciplinary team, communicate with families and payors, and advocate for patient rights while ensuring timely, quality transitions of care across hospital settings.

Qualifications

• Master's degree required.
• 2+ social work experience preferred.
• ACM/ CCM certification preferred.

Responsibilities

• Provide grief counseling, discharge planning support, and care coordination.
• Assess discharge needs for inpatients, observation, and ED patients.
• Review medical records to integrate clinical, social, and financial factors into care plans.
• Develop discharge plans with contingencies and coordinate post-acute care.
• Communicate with post-acute services and payers for seamless transitions.
• Participate in multidisciplinary rounds to optimize patient outcomes.
