You will review records, coordinate post-acute services, and advocate for patients and families within a multidisciplinary team.

Qualifications

• Requires MSW and state licensure or eligibility.
• 2+ care management experience preferred.
• 2+ social work experience required.

Responsibilities

• Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate.
• Assesses discharge planning needs for inpatient, observation, and emergency departments to avoid delays.
• Reviews medical records and integrates clinical, social, and financial factors into discharge plans.
• Develops discharge plans with contingency plans for post-acute care and community resources.
• Communicates with post-acute care services to ensure continuity of care and complete medical records.
• Participates in multi-disciplinary rounds to optimize patient status, resources, and timely transition of care.
• Organizes care conferences and documents evaluations, plans, and patient rights during hospitalization.
• Communicates with payors for post-acute care authorizations as needed.
• Assesses readmitted patients for reasons related to readmission.
