Job summary

Qualifications

• Master's degree required.
• 2+ years care management experience preferred.
• 2+ years social work experience required.
• ACM/CCM licensure preferred.

Responsibilities

• Provide grief counseling, crisis intervention, and discharge planning support.
• Assess discharge needs for inpatient, observation and ED patients.
• Develop discharge plans and coordinate post-acute services.
• Communicate with payors for post-acute authorization.
• Lead multidisciplinary rounds and advocate for patient wellbeing.
