Job summary

UChicago Medicine AdventHealth in La Grange, IL is seeking a dedicated Care Manager/Social Worker to provide discharge planning, care coordination, and post-acute planning for inpatient, observation, and emergency department patients.

You will assess medical, psychosocial, and financial factors, collaborate with multidisciplinary teams, educate families, and advocate for patient rights while navigating payer authorization and community resources to ensure timely, safe transitions.

Qualifications

• Master's degree required.
• 2+ care management experience preferred.
• 2+ social work experience required.
• Additional licensure or certification requirements may apply depending on unit/state.

Responsibilities

• Provides discharge planning assessment and development for inpatient, observation, and emergency department patients.
• Reviews medical records to incorporate clinical, social, and financial factors into transition of care.
• Develops discharge plans with contingency plans and coordinates post-acute services and community care.
• Communicates with and educates patients and families about emotional, social, and financial impacts of illness and care decisions.
• Organizes and facilitates patient and family care conferences with the multidisciplinary team.
• Documents discharge planning evaluations, barriers, and progression of care according to standard work.
• Advocates for patient rights and pays attention to authorization needs with payors.
• Participates in multidisciplinary rounds to review patient status and resources for discharge.
