Job summary

Advocate Health seeks a social work professional to perform comprehensive assessments, discharge planning, and care coordination within a multidisciplinary team. The role emphasizes patient-centered planning, regulatory compliance, and effective communication with patients, families, and payers.

The position requires MPH/LSW/LCSW eligibility in Illinois with a focus on hospital-based case management and post-acute coordination, in a 40-hour work week at Advocate Christ Medical Center.

Qualifications

• MSW from CSWE-accredited program required or preferred as applicable.
• Active licensure/eligibility to practice social work in relevant states.
• Experience in healthcare social work and discharge planning is desirable.

Responsibilities

• Conducts complete assessments and initiates care plans in a timely manner.
• Provides psychosocial assessment, discharge planning and follow-up.
• Collaborates with multidisciplinary teams to coordinate post-acute care.
• Handles CMS regulatory notices and communication with payers when needed.
• Maintains knowledge of community resources and applicable regulations.
• Educates patients and families on advance directives and care options.
• Ensures safe and appropriate care across the care continuum.
