Job summary

OU Health seeks a Care Management Registered Nurse (RN) in Adult Discharge Planning to coordinate care for selected patient populations, assess needs, and develop individualized plans in collaboration with a multidisciplinary team.

The role emphasizes education for patients and families, communication across providers, and ensuring regulatory and payer compliance to promote quality outcomes and reduce readmissions.

Qualifications

• Associate's degree in nursing is required for Level 1; Bachelor's degree preferred/required for higher levels.
• RN license issued by state board or compact, with active CPR/BLS.
• Experience in care management strongly preferred for Levels 2 and 3.

Responsibilities

• Conduct comprehensive assessments of patients' health status and care needs.
• Develop individualized care plans with the care team and families to ensure continuity of care.
• Educate patients and families about conditions, treatment plans, medications, financial expectations, and self-care.
• Coordinate communication among patients, families, providers, and community resources.
• Evaluate effectiveness of care plans and adjust as needed.
• Identify opportunities to improve utilization and reduce cost without compromising quality.
• Collaborate with insurers and social workers to obtain timely authorizations.
• Monitor patient outcomes and care delivery processes for improvement.
