Job summary

OU Health in Oklahoma City is seeking an Adult Care Management Registered Nurse, RN - PRN to assess, plan, implement, coordinate, monitor and evaluate care for selected patient populations. Qualifications

• Associate's degree in nursing required.
• 1 year of Care Management experience.
• Current RN license in Oklahoma (or compact) and BLS certification.

Responsibilities

• Assess patients' health status, medical history and care needs.
• Develop and implement individualized care plans with the care team.
• Educate patients and families on conditions, plans, medications and self-care.
• Coordinate communication among patients, families, providers and community resources.
• Monitor and evaluate care plans and adjust as necessary.
• Evaluate utilization and identify opportunities to improve efficiency and cost-effectiveness.
• Ensure regulatory compliance and participate in quality improvement initiatives.
• Collaborate with insurers, social workers and other stakeholders for authorization and coverage.
• Promote patient safety and reduce readmissions through care planning enhancements.
