Job summary

OU Health is seeking a Care Management Registered Nurse (RN) for Adult Discharge Planning. The role focuses on coordinating care, planning and evaluating services to meet patient needs while ensuring quality outcomes and satisfaction.

The RN will assess patients, develop care plans with the team and families, educate on health conditions and medications, and facilitate communication across care settings. Shift: M-F Days. Benefits and relocation support may be available.

Qualifications

• Associate's degree or higher in nursing required.
• Minimum 0–3 years nursing experience required, care management experience preferred.
• Current Registered Nurse License (OK or eNLC) and Basic Life Support from the AHA required.

Responsibilities

• Assess, plan, implement, coordinate, monitor, and evaluate patient care and services.
• Develop individualized care plans with the interdisciplinary team and families for continuity of care.
• Educate patients and families about health conditions, treatment plans, medications, and self-care strategies.
• Coordinate communication between patients, families, providers, and community resources to optimize outcomes.
• Evaluate care plans and adjust to improve outcomes and cost-effectiveness.
• Advocate for appropriate resource allocation while adhering to regulations and reimbursement criteria.
• Collaborate with insurance providers, social workers, and other stakeholders to ensure timely services.
• Monitor patient outcomes and care delivery to identify areas for improvement.
