Job summary

UMass Memorial Health is seeking an RN with discharge planning and case management experience to join our Care Coordination team. You will assess patient needs, develop plans of care, and coordinate resources across the care continuum.

Responsibilities include collaboration with physicians and payers, utilization management, and data collection to drive program improvements. Massachusetts RN licensure is required, and the role supports a 40-hour week on day shift.

Qualifications

• Massachusetts licensure as a registered nurse.
• Three (3) years clinical experience in discharge planning or case management.
• Ability to use computer-based clinical data systems.
• Excellent organizational and communications skills.
• Utilization management knowledge preferred.

Responsibilities

• Assess patient needs and develop care plans with appropriate resources.
• Coordinate care across the continuum and communicate with payers.
• Perform admission and concurrent reviews; liaise with case managers.
• Support discharge planning, monitor outcomes, and adjust plans as needed.
• Participate in quality improvement and regulatory compliance.
