Job summary

St. Elizabeth Physicians is seeking a Care Management Referral Coordinator to manage the Care Management Referral Line, coordinate referrals, and support SEP Grant Funded Transportation Assistance for patients in the community.

The role requires strong communication, critical thinking, and organizational skills with a focus on improving health outcomes and care coordination. A baseline Bachelor’s degree or equivalent experience is expected.

Qualifications

• Bachelor’s degree in Human Services or related field; or an associate’s degree with applicable experience.
• Minimum 2+ years in health-related or community resources.
• Care Management experience preferred.

Responsibilities

• Answer incoming calls to Care Management Referral Line and provide resolution.
• Review and route incoming referrals based on chart review and patient needs.
• Conduct initial assessments to determine appropriate team member routing.
• Identify and create appropriate referrals and coordinate services.
• Educate patients with resources, timelines, and goals for accountability.
• Coordinate SEP Grant Funded Transportation Assistance and document in central locations.
• Schedule and manage appointments for Care Management Team.
• Connect patients with community resources and ensure timely follow-up.
• Maintain HIPAA-compliant communication and collaboration with care team.
• Participate in department meetings and care collaboratives to improve outcomes.
