Sevita, through its ResCare Community Living unit in North Carolina, seeks a skilled Care Manager to partner with clinical teams and build tailored plans for individuals with IDD. You will develop assessments and care plans, monitor progress, and coordinate services to improve health and independence.

The role emphasizes care coordination across medical, psychosocial, housing, and community supports, with strong emphasis on documentation, team collaboration, and patient-centered outcomes.

Qualifications

• Two years of experience as a Care Manager, Case Manager, or Care Coordinator preferred.
• License, provisional license, certificate, registration, or permit in a human services field.
• Master’s degree in a human service field with one year of post-graduate IDD experience.
• Bachelor’s degree in a human service field with two years of post-bachelor IDD experience.
• LTSS/care management experience two years in addition to the IDD requirements.

Responsibilities

• Develops and monitors Tailored Care Management and ISP plans.
• Coordinate care team meetings and ensure care plan implementation.
• Facilitate transitions and link members to medical, housing, and community services.
• Ensure timely electronic documentation of all interactions.
