Job summary

ResCare Community Living, a Sevita company, is seeking a Care Manager to collaborate with clinical teams and oversee tailored care plans and ISPs for individuals with intellectual and developmental disabilities in North Carolina.

You will coordinate comprehensive assessments, facilitate care team meetings, and ensure timely documentation and service delivery in line with state requirements and Sevita standards.

Qualifications

• Two years of experience as a Care Manager, Case Manager, or Care Coordinator preferred.
• Licensure or degree requirements per NC state rules.
• Experience with LTSS and IDD population is preferred.

Responsibilities

• Develops care plans and ISPs based on comprehensive assessments for assigned caseload.
• Coordinates care team meetings and informs all members of schedules and updates.
• Maintains up-to-date electronic records and documents billable events per guidelines.
• Engages member/family to ensure services meet health and quality of life goals.
• Supervises care management extenders and coordinates with medical, behavioral, and community providers.
