Qualifications

• Active nursing licensure (RN/LPN/EMT/PharmD or international equivalent)
• 5+ years nursing/case management experience
• 2+ years leadership experience
• Experience in transitions of care and senior populations
• Knowledge of chronic conditions and care interventions
• Experience in value-based/managed care settings
• Proficiency in analyzing data trends and reporting
• Willingness to travel to clinics and community sites
• Strong communication and stakeholder management skills
• Ability to set SMART goals and drive accountability

Responsibilities

• Lead daily operations of the Care Integration Team (CIT).
• Oversee program goals across productivity, quality, and adherence to processes
• Monitor team performance with dashboards and metrics
• Develop action plans to meet targets and improve outcomes
• Hire, onboard, train, and retain CIT members
• Manage high-risk patient caseload and care planning
• Collaborate with market leaders to optimize performance and reduce avoidable utilization
• Lead regular market leader performance reviews
• Foster a high-performing team culture and professional growth
• Maintain partnerships with community organizations and health systems
