Bon Secours Mercy Health seeks a Manager, Care Management to oversee daily operations of the Care Management Department across multiple locations. The role blends clinical and financial aspects to optimize patient outcomes and resource use.

You will lead care transitions, LOS management, and readmission prevention while aligning with population health efforts and standards for care management services. A BSN or related degree is required with 3 years of acute hospital care management experience.

Qualifications

• Bachelor of Science in Nursing or Social Work (required).
• Masters in Nursing, Social Work, or Healthcare Administration (preferred for BSMH, required for RSFH).
• 3 years of recent acute hospital care management experience (required).

Responsabilités

• Provides leadership and oversees day-to-day operations of the Care Management Team.
• Advocates and educates regarding right care and timely transitions.
• Utilizes systems to support care management across the continuum and collabo rates with population health initiatives.
• Directs utilization of personnel resources and helps develop annual departmental goals.
• Participates in budget development and cost reduction efforts.
