Job summary

Qualifications

• Minimum 1 year of experience in substance abuse treatment or related healthcare.
• RADT certification is required.
• CADC-I certification preferred.
• Knowledge of FMLA, disability, and legal documentation processes.
• Familiarity with confidentiality laws (State & Federal) and regulations.

Responsibilities

• Complete Case Management Needs Assessments and SNAP forms for all clients.
• Create and implement Case Management Treatment Plans tailored to each client.
• Develop comprehensive Aftercare Plans to support long-term recovery.
• Facilitate weekly case management groups.
• Assist clients with FMLA, return-to-work documentation, and legal forms.
