Job summary

Qualifications

• High school diploma or equivalent required.
• CDCA credential required.
• QMHS III qualification or ability to meet requirements as applicable.
• Minimum of one year of relevant behavioral health/substance use treatment or case management experience.
• Working knowledge of community resources and social-service systems.
• Strong crisis intervention, conflict-resolution, and de-escalation skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide case management services to clients in substance use disorder and behavioral health programs.
• Assess client needs and barriers related to housing, employment, transportation, healthcare, mental health, legal matters, benefits, education, and other areas affecting treatment.
• Develop and maintain individualized case management/service plans based on identified needs and goals.
• Provide ongoing support and service coordination to help clients meet goals.
• Connect and refer clients to community resources (housing, employment, mental health, medical care, transportation, benefits, education).
• Coordinate care with therapists, peer support specialists, medical providers, and other treatment team members.
