Qualifications

• Willing and able to travel within the Northeast Ohio region up to 50% of the time.

Responsibilities

• Provide care coordination and education to support member health outcomes.
• Collaborate with case management teams and care providers.
• Document care plans and monitor progress in accordance with regulatory guidelines.
• Serve as a single point of contact for members and assist with gaps in care.
