Job summary

CVS Health is seeking a Case Management Coordinator to support Ohio Medicaid members with comprehensive care coordination. The role requires traveling to member homes and other locations within Northeast Ohio, up to 50% of the time, and working a M-F schedule with flexibility to meet member needs.

The position emphasizes evaluation, coordination of care plans, and collaboration with multidisciplinary teams to ensure quality outcomes for children and families served by OhioRISE.

Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers, keyboard navigation, and MS Office Suite.

Responsibilities

• Be clinically and culturally competent with training to manage complex cases in the community.
• Coordinate and monitor care plans; evaluate needs and eligibility.
• Collaborate with care teams to improve care quality and outcomes.
• Provide coaching and education to empower members in health decisions.
• Remediate gaps in care and act as a single point of contact for members.
