Job summary

Lancaster County is seeking a care manager to provide care management for OPTIONS services and those on the aging waiting list. The role involves working with consumers and families to assess needs and develop a comprehensive ISP, with coordination of services from formal and informal providers, often via home visits and phone contacts.

Responsibilities include documentation, data entry, and ensuring services meet consumer needs while following agency standards and HIPAA requirements.

Qualifications

• Current knowledge of HIPAA regulations and their interpretation is maintained and adhered to.
• Give and receive information and feedback in order to provide a more complete result.
• Demonstrate respect for and awareness of others, good judgment in responding to conflict, and flexibility to adapt to changes in one’s work environment.
• Understand and anticipate customer needs.

Responsibilities

• Provides care management for consumers receiving OPTIONS services, as well as those on the waiting list.
• Works with consumer and family to assess need, develop and implement an Individual Service Plan (ISP).
• Completes comprehensive ISPs, documents per requirements, and coordinates services to ensure safety and needs are met.
