The position emphasizes collaboration with social workers, clinicians, and external partners to optimize referrals and discharge planning, aiming to improve the patient admission experience.

Qualifications

• Required: LPN licensure per state or Bachelor of Social Work (BSW).
• BLS certification/listing may be required as applicable.

Responsibilities

• Provides collaborative administrative support for social workers and Case Managers by completing documentation and contacting patients to reduce staff workload and increase office efficiency.
• Prepares documents such as referral forms, using proper grammar and spelling to communicate with other providers.
• Corresponds with patients, caregivers, insurance companies, post-acute vendors and community agencies under leadership of a Case Manager or Clinical Social Worker (LSW).
• Informs departmental leaders about potential gaps in coverage and advises on resource needs.
• Develops strong business relationships with Cone Health's post-acute network partners across care settings (SNF, LTAC, IRF).
• Networks and contributes to business development with post-acute partners, VBCI leadership, and Cone Acute facilities for referrals and educational opportunities.
• Collaborates with Cone Health acute care facility and post-acute resources, and community partners to deliver exceptional patient admission experience.
