Job summary

Bon Secours is seeking a Care Management Assistant – St. Mary's Hospital - PRN to support care management staff and help coordinate discharge planning for patients. The role requires strong people skills and compassion, with exposure to discharge procedures and patient communications.

Qualifications

• High School Diploma or GED required.
• Some college coursework preferred.
• BLS Basic Life Support – American Heart Association required.

Responsibilities

• Assist care management staff with daily duties and discharge planning.
• Explain observation status notifications to patients in observation status within 24–36 hours of admission.
• Collaborate with Care Managers and Social Workers; assist with faxing, mail, phone messages, and other tasks.
• Provide updates on approvals/denials to Care Managers and Social Workers as information is received.
• Assist with non-complex discharge planning and transitions under supervision.
