Bon Secours is seeking a Social Worker Care Manager at St. Mary's Hospital Richmond to coordinate discharge planning and provide patient-centered support. The role includes clinical supervision of peers and students, with emphasis on holistic, biopsychosocial care and collaboration across the care team.

Required: Bachelor of Social Work, and VA social work license; BLS certification. Experience in acute/ambulatory settings and a commitment to compassionate, respectful care is essential.

Qualifications

• Bachelor of Social Work (required).
• Master’s degree in social work or healthcare related field (preferred).
• Licensed as a Social Worker in state of practice (required, VA preferred).
• ACM/CCM certification preferred.

Responsibilities

• Identify and prioritize patients in need of social services.
• Complete all necessary documentation in patient records.
• Plan post-hospital care with patient, family and care team.
• Follow discharge planning and readmission prevention processes.
• Promote timely access to testing and treatment, addressing barriers.
