Job summary

St. Mary’s Medical Center is seeking a full time Discharge Planning Specialist to support Care Facilitators and Social Workers in coordinating timely discharge planning. The role involves coordinating home health, DME, rehab placement, and other community resources to ensure safe transitions from hospital care.

Requirements include a high school diploma, preferred post-secondary training, BLS certification, and strong communication skills to interact with patients, families, and healthcare teams.

Qualifications

• High School Diploma is required. Junior College/Technical School preferred.
• Education and training in hospital healthcare transitional care, and discharge planning beyond the hospital setting, preferred.
• BLS required. Working knowledge of medical terminology. Excellent communication skills-verbal and written ability to work with patients, families, physicians, and other hospital departments.

Responsibilities

• Assist the Care Facilitator and/or Social Worker with discharge planning responsibilities to promote timely and safe discharge services.
• Facilitate and coordinate discharge services such as home health, DME, Rehab placement, and other community resources or agencies.
