NeighborHealth Center, Inc. seeks a Quality Care Coordinator to identify and close preventive care gaps and manage chronic disease through outreach and care coordination.

The role collaborates with clinical teams to ensure patients receive recommended services and complete follow-up appointments. This on-site position involves patient contact via calls, texts, and patient portal, maintaining care gap tracking, generating quality reports, and supporting continuous improvement in preventive care

Qualifications

• Bilingual in English and Spanish preferred.
• Computer literacy in internet use and Windows environment, including Outlook, Word, and Excel.
• Proficiency with electronic health records, registries, and quality reports.
• Ability to explain preventive care to patients in clear language and encourage follow-through.
• Strong organizational skills with the ability to manage large patient lists and prioritize outreach.
• High attention to detail and accuracy in documentation.
• Ability to recognize when a patient needs a licensed clinician and escalate promptly.
• Ability to cultivate inclusive and equitable working relationships with co-workers and community members.
• Ability to advocate for individuals of all ethnicities, genders, ages, and backgrounds.

Responsabilités

• Identify and track preventive care gaps using registries and EHR.
• Outreach patients via calls/texts/portals to schedule services.
• Collaborate with clinical teams to close gaps and improve quality measures.
• Maintain tracking lists of patients with open gaps and outreach status.
• Generate quality reports and monitor outreach and appointment outcomes.
• Support escalation procedures and care coordination workflows.

Formation

High school diploma or equivalent

Associate degree in health-related field preferred

CMA/RMA/CCMA certification preferred
