Job summary

The Wright Center for Graduate Medical Education is seeking a Care Gap Closure & Pre-Visit Planning Specialist to enhance preventive care, chronic disease management, and visit efficiency. You will proactively outreach to patients, coordinate necessary services, prepare charts, and ensure the care team has up-to-date information before encounters.

Work closely with clinicians, nursing, care management, population health, and practice operations to close care gaps and optimize patient encounters.

Responsibilities

• Identify and close care gaps identified in patient registries and quality reports.
• Conduct comprehensive pre-visit planning for assigned patients.
• Support clinical teams by making high-priority patient needs visible and actionable.
• Coordinate necessary screenings, tests, immunizations, referrals, and documentation.
• Maintain accurate documentation of outreach and care gap activities in the EHR.
• Participate in daily or weekly population health workflows and team huddles.
