Job summary

Atria Health in New York City is seeking a Care Coordinator to join our flagship NYC team. You will use superior administrative and customer service skills to support our members and redefine care coordination.

Qualifications

• Bachelor’s degree is required.
• 5+ years of customer service experience in clinical, hospitality, or membership roles.
• Protect confidential information and maintain privacy (HIPAA).

Responsibilities

• Serve as the primary contact for member scheduling and administrative needs.
• Support physicians and specialists and coordinate their panels.
• Collaborate with Care Coordination, Hospitality, and Clinical Operations to deliver integrated care.
• Provide high-level admin support: phones, meetings, calendars, conference calls.
• Maintain backend systems and ensure timely communication of updates.
• Procure medical records and follow-up notes from external practices.
• Use EMR and databases to provide records with strict confidentiality.
• Maintain organization and proactive communication to keep teams on schedule.
