Job summary

Terros Health is seeking a Families FIRST - Care Coordinator for our Campbell Ave Counseling Center in Tucson, AZ. This full-time role collaborates with DCS, clinical staff, and community partners to engage families, streamline services, and remove barriers to treatment and reunification.

Qualifications

  • Associates or higher in human service/behavioral health with relevant experience.
  • CPR/First Aid certification within 60 days of start date.
  • Arizona driver’s license and ability to meet Terros Health driving policy requirements.
  • Must pass background check and Arizona DCS Central Registry clearance.
  • At least 3 years working with multi-problem families in substance abuse, child welfare, or behavioral health.

Responsibilities

  • Conducts New Client Checklist activities and documents in the patient record.
  • Completes Short-Term Plan to ensure AFF services access within 3 business days.
  • Leads monthly Service Plan review with the Service Team and updates quarterly for DCS submission.
  • Performs outreach and follow-up to ensure resources and supports are in place.
  • Participates in CFT, ART, TDM, SENSE, and DCS/JOBS Staffing meetings.

They will also empower families by providing resources and skills to show stability in their homes.
