Housing Solutions of New York is seeking a Care Coordinator Supervisor (EHP) to join our team!

About the Role:

The Care Coordinator Supervisor oversees the case management staff to ensure effective service delivery to families and individuals in the Emergency Housing Provider Management Program (EHPMP). Responsibilities include supervising case managers, monitoring documentation, conducting psychiatric evaluations, and supporting clients' housing applications.

Responsibilities:

1. Review case file documentation for compliance and quality.
2. Supervise case managers conducting intakes, assessments, and referrals.
3. Orient new staff to the program and their roles.
4. Ensure clients are linked to community services and track these linkages.
5. Assist with data collection and professional development planning.
6. Monitor entitlements and maintain confidential records.
7. Coordinate with community providers and conduct psychiatric evaluations.
8. Follow up on referrals based on evaluations.
9. Perform additional duties as assigned.

Qualifications:

Minimum 1-3 years supervisory experience, excellent writing skills, knowledge of homeless issues, criminal justice, HIV/AIDS, substance abuse, and mental health assessment using DSM 5.

Position Scope: Supervise case management and conduct psychiatric evaluations to support clients' housing stability.

• Pay Range: $70,000/year
