The HARP Care Coordination Supervisor will be responsible for supervising Care Coordinator operations within the Health Home Division. Responsibilities include monitoring the departmental phone queue, conducting training for clinical staff, supporting daily operations, and enhancing communication between clinical and non-clinical areas to meet member and employee needs.

Responsibilities include:
  1. Providing guidance within the HARP Department regarding processes and workflows
  2. Promoting a multidisciplinary approach supporting HARP care operations
  3. Overseeing clinical program training and updating departmental resources
  4. Adhering to clinical standards of care and collaborating with providers
  5. Providing crisis intervention when necessary
  6. Maintaining quality review logs and conducting quarterly audits
  7. Collaborating with management to meet program goals and initiatives
  8. Supporting Care Management staff as needed
  9. Following best practices and departmental guidelines
  10. Performing other duties as required
Qualifications:
  • MSW/MA/MS Master’s Degree or equivalent
  • LMSW, LCSW, LMHC, LMFT, RN preferred
  • Two years’ experience in Behavioral Health/Substance Abuse
  • Prior supervisory experience
  • At least 3 years in managed care
  • Knowledge of the Collaborative Care Model and chronic condition management
  • Experience with HCS/MAPP systems preferred
  • Excellent communication skills and ability to multitask
  • Leadership and professionalism
  • Proficiency with Word and Excel
