Job summary

Qualifications

• 1+ years of previous experience required.
• BLS required.
• CPI required within 30 days of hire.

Responsibilities

• Provide face-to-face substance abuse psychosocial assessments
• Daily review and update of completing assessments, and other duties assigned by program director
• Facilitate patient referrals for additional services consistent with assessed needs and physician recommendation
• Meet patients for follow up reviews or updates as needed
• Provide brief interventions, stabilization services and supportive counseling to patients
• Care coordinator will provide patients medication education print outs that are focused on understanding illness, symptom management and medication compliance
• Complete and file documentation as required in timely manner
