Job summary

Haymarket Center is seeking a Care Coordinator to join our team. The role involves collaborating with medical providers and the Care Coordination team to deliver individualized, evidence-based recovery services to patients identified in hospital EDs and MSUs.

The ideal candidate will hold CACD/CRADC/MAAT/MISA certification from IAODAPCA, have experience with screenings, assessments, education, and case management, and possess a high school diploma or GED. A valid driver's license is required.

Qualifications

• CACD, CRADC, MAAT or MISA certification from IAODAPCA.
• Two years prior experience working with individuals with substance use disorders, completing screenings and assessments.
• Experience providing healthcare education and completing case management activities.
• One year experience facilitating therapeutic or educational groups.
• High School diploma or GED.
• Experience working with culturally diverse populations.
• Must possess a valid driver's license and driving privileges through agency insurance.

Responsibilities

The Care Coordinator will work closely with medical providers and the Care Coordination team. The Care Coordinator provides individualized and evidence based substance use recovery services to patients identified in various hospital Emergency Departments and Medical Stabilization Units.
