Job summary

PrimeCare Health seeks a Care Coordinator for Substance Use Disorder in Chicago. The role coordinates MAT-related treatment, engages patients in care, and supports referrals to social and mental health services. Excellent communication and teamwork are essential for success in a fast-paced clinic.

Responsibilities include tracking outcomes, coordinating with PCPs and therapists, and participating in daily huddles to assess progress and adjust plans as needed. Prior experience in SUD is preferred.

Qualifications

• Associate’s or Bachelor’s degree in relevant field, 1–3 years related experience preferred.
• CADC or prior work with substance use disorder patients preferred.
• Eligibility for NHSC or State Loan Repayment programs.
• 27 days PTO per year, accrued each pay period; 3 personal days; 1 floating holiday; 8 paid holidays.

Responsibilities

• Provide proactive outreach to MAT patients to engage in their own health.
• Coordinate treatment care with MAT PCP and other behavioral health providers.
• Track follow-up and outcomes; document encounters and re-engage patients.
• Facilitate treatment plan changes with PCP and providers; advise referrals.
• Participate in daily huddle and discuss patient progress.
• Facilitate referrals to social services and other care settings.
• Complete relapse prevention plans for patients in remission.
• Attend ongoing training and collaborate to ensure patient support services.
