Mental Health Cooperative in Nashville, TN seeks a Care Coordinator for Safety Net services to provide coordination of mental health care and navigation of community resources. You will assess needs, connect clients with services, and advocate for appropriate care within a collaborative clinic setting.

The role emphasizes in-home/community visits, thorough documentation, timely progress notes, and adherence to Joint Commission competencies. A health-related bachelor’s degree is required.
