Mental Health Cooperative in Manchester, TN, is hiring a Care Manager I to provide comprehensive care management for children, youth and adults with complex needs. The role emphasizes collaboration with an interdisciplinary team and community partners to help clients achieve healthy, stable lives.

You will conduct home, school or community visits, assess needs, and develop individualized care plans while maintaining timely documentation and a strong focus on outcomes.

Qualifications

• Bachelor’s degree in counseling, psychology, social work or related behavioral health field.
• Fifteen college hours in behavioral health if degree is Criminal Justice.
• Valid Tennessee Driver’s License and clean MVR.

Responsibilities

• Provide comprehensive care management services to children, youth and families.
• Engage monthly with consumers to assess needs and update care plans.
• Complete community visits to evaluate needs and coordinate services.
• Address mental and physical health, and social determinants of health.
• Document activities timely and maintain policy-compliant records.
• Collaborate with team and community partners to support goals.
