Mental Health Cooperative in Nashville seeks a Care Coordinator, Adult (Cooperative Recovery) to provide comprehensive care coordination for buprenorphine treatment and related services. You will collaborate with consumers, families, and community partners to assess needs, develop care plans, coordinate supports, and monitor progress in a patient-centered, respectful manner.

This role emphasizes timely documentation, crisis planning, and adherence to agency guidelines while promoting recovery

Qualifications

• Bachelor’s degree in a health-related field (counseling, psychology, social work or related).
• Criminal justice degree holders must have 15 college hours in behavioral health.
• Knowledge of HCV/HIV, infectious disease, addiction, CPR/First Aid, harm reduction.
• Ability to handle complex, highly emotional and stressful situations.
• Client- and mission-centered approach with strong listening skills.
• Valid Tennessee Driver’s License and acceptable MVR/background.

Responsabilités

• Assess consumer needs and establish an agreed plan to address them.
• Coordinate care with community supports, providing home/school/community-based support.
• Complete care/crisis plans and assessments within defined timeframes.
• Initiate referrals and linkages to resources and arrange transportation.
• Advocate for consumers and assist with appointments and barriers to attendance.
• Educate on gaps in care and develop plans to close gaps and manage health proactively.
• Educate on ED/IP usage and ensure proactive health management.
• Ensure daily psychiatric, medical, and environmental needs are coordinated.
• Maintain timely documentation and build community relations.
