• Bachelor’s Degree in a health-related field of counseling, psychology, social work or other behavioral health field.
• Candidates with a bachelor’s degree in criminal justice must have fifteen college level semester hours of coursework in behavioral health.
• Knowledge of HCV/HIV/ pathogens and infectious disease, Addiction, CPR/ First Aid, Harm Reduction Models and Best Practices.
• Ability to be client and mission centered.
• Excellent interpersonal skills with ability to effectively listen and offer solutions without passing judgement.
• Valid Tennessee Driver’s License, acceptable MVR and background.

Responsabilidades

• Assess consumers' basic needs and establish an agreed-upon plan to address pertinent needs.
• Meet with consumers and gather releases of information to coordinate care with community supports.
• Provide support to consumers in their home, at school, at court, at appointments in office, and in community settings.
• Complete comprehensive care plans and risk assessments within 30 days of admission and every 6 months thereafter.
• Coordinate referrals and linkage to internal and community resources.
• Advocate for consumers and assist with transportation and appointment reminders.
• Document timely and accurate information per agency guidelines.
• Engage in supervision and team meetings to ensure coordinated care.

Educación

Bachelor’s Degree in a health-related field

Criminal justice degree with 15 credits in behavioral health
