Job summary

Mental Health Cooperative in Nashville, TN seeks a Care Coordinator, Adult (Cooperative Recovery) to provide comprehensive care coordination and management for individuals receiving low-threshold buprenorphine treatment.

You will assess needs, coordinate plans with community supports, support clients at home and in the community, document promptly, and advocate for access to appointments and services to promote recovery.

Qualifications

• Bachelor’s degree in health-related field (counseling, psychology, social work or related).
• Knowledge of HCV/HIV, pathogens, infection control and harm reduction.
• CPR/First Aid certification preferred.
• Ability to coordinate care with multiple community supports.
• Strong interpersonal skills and ability to handle stressful situations.

Responsibilities

• Assess consumers’ basic needs and establish an agreed care plan.
• Coordinate care with internal and community supports, in home, school, or clinic.
• Provide education on gaps in care and action plans to address them.
• Initiate referrals and ensure follow-up appointments and transportation.
• Maintain timely documentation and participate in team supervision.
