Ez Health is seeking Care Coordinators in the United States to provide remote, ongoing support to a small group of older adults. You’ll help with appointments, transportation, medications, and benefits, staying engaged over months and years via phone and web tools.

Roles are part-time or full-time, 28–40 hours per week, with pay ranging from $28 to $38 per hour depending on experience. Training is provided, with opportunities to grow within healthcare.

Getting older shouldn't mean getting lost in the healthcare system. But for many seniors, one doctor's appointment turns into three referrals, a new prescription the pharmacy can't fill, an insurance letter nobody understands, and no ride to get there. EZ Health exists to help with that, and we're looking for Care Coordinators to do the work.

The Role

You’ll be the main point of contact for a small group of older adults, usually 20 or fewer. You’ll work with them by phone over months and years, not just one call. When something gets in the way of their health or independence, you’ll figure out what's going on, line up the right help, and stay on it until it's resolved.

This is people work and problem-solving work in equal parts. One day you might help a client get ready for a cardiology appointment. The next, you might track down a meal delivery program in their county, sort out a prescription that's stuck between the doctor and the pharmacy, or find a ride after the usual transportation option falls through.

A Typical Week Might Include

• Checking in by phone with each of your clients to see what's new
• Talking through what's worrying them and asking the questions that get to the real issue
• Getting a client ready for an upcoming appointment, then calling afterward to go over what the doctor said
• Calling doctors' offices, pharmacies, insurance companies, and community organizations on a client's behalf
• Researching local resources like transportation, food programs, prescription assistance, and senior services
• Explaining a benefit or a next step in plain language, as many times as it takes
• Coming up with a plan B when the normal process doesn't work
• Writing clear notes so the rest of the team can pick up where you left off
• Closing the loop with clients so they always know what happened

Who Does Well Here

Healthcare experience is welcome but not required. Many of our best coordinators come from customer service, social services, hospitality, teaching, or caregiving. We’ll train you on the healthcare side. What matters most is how you treat people and how you handle problems.

You’ll probably enjoy this role if you:

• Like older adults and are happy to spend most of your day on the phone
• Don’t mind picking up the phone and making the first call
• Are naturally curious and keep asking until something makes sense
• Enjoy research and finding answers that aren't easy to find
• Can make a complicated topic simple without talking down to anyone
• Stay steady and warm when someone is anxious, upset, or confused
• Keep track of lots of moving pieces and don't let things fall through the cracks
• Take initiative without needing to be reminded
• Are comfortable working with email, Google Docs, Google Sheets, and web-based tools
• Want a job where you can point to the people you’ve helped
