Job summary

Renewal Falls Recovery Center in Cedar Falls, IA is seeking an Intake Coordinator (Care Coordinator) to guide patients from admission through discharge, coordinating care with treatment teams and community resources. This full-time role requires a Bachelor’s degree in social work or related field, experience in behavioral health, and strong organizational skills.

You’ll support admissions, assessments, planning, and follow-up to strengthen recovery.

• Experience coordinating intake, case management, or discharge planning in behavioral health.

Responsibilities

• Assist with admission and intake, including orientation and documentation.
• Identify patient needs and coordinate care across providers.
• Participate in interdisciplinary treatment meetings and progress updates.
• Assist with treatment planning and goal review.
• Coordinate referrals to healthcare, outpatient treatment, and community resources.
• Maintain timely documentation in the electronic medical record.
• Support authorization and utilization review processes.
