• High school diploma or GED required.
• Associate or bachelor’s degree in social work, human services, psychology, behavioral health, sociology, criminal justice, or related field preferred.

• Ability to maintain confidentiality and professional boundaries.

Responsibilities

• Perform comprehensive case management assessments to identify patient needs, barriers and supports.
• Develop and maintain individualized case management goals with the treatment team.
• Begin discharge and continuing-care planning early in treatment.
• Coordinate referrals and connections to community resources and services.
• Assist patients with housing, transportation, employment resources, education, insurance, benefits, and healthcare access.
• Maintain accurate EMR documentation and ensure releases of information when needed.
