Job summary

Renewal Falls Recovery Center in Cedar Falls, IA seeks a compassionate Substance Use Disorder Case Manager to help patients overcome barriers to long-term recovery. You will work with the treatment team to create practical recovery plans, coordinate housing, transportation, and benefits, and support ongoing treatment transitions.

Qualifications

Responsibilities

• Perform comprehensive case management assessments to identify needs and barriers.
• Develop individualized goals with patients and treatment team.
• Plan discharge and continuing-care early in treatment stages.
• Coordinate referrals to housing, transportation, employment, education, and benefits.
• Assist patients with obtaining IDs, insurance information, and documentation.
• Maintain timely case notes in the EMR and ensure information accuracy.
