Job summary

Responsibilities

• Coordinates discharge planning and aftercare with treatment team and referrals.
• Engages clients and families to explain options and supports service plans.
• Ensures timely contacts with referral sources and updates on service plan.
• Assists with eligibility screenings and applications for entitlements.
