Qualifications

• Licensed Social Worker or Registered Nurse licensed to practice in Minnesota is required.
• Minimum of one year of experience with assessments and knowledge of community resources.
• Valid driver's license is required.
• Must pass background study and motor vehicle check.
• Strong written and verbal communication skills.
• Computer skills including basic knowledge of Microsoft Office Suite.
• Must be detail oriented, highly organized, and possess analytical and problem-solving skills.
• Able to work independently and participate actively in a team.
• Able to implement strategies to enhance delivery of services to culturally diverse populations.
• Able to recognize boundaries and exhibit patience and self-control in a dynamic environment.

Responsibilities
