Job summary

Lutheran Social Service of Minnesota is seeking a Care Coordinator to perform initial and annual assessments, develop a Plan of Care, and coordinate services for enrollees. The role focuses on culturally appropriate, person-centered care and improving access to resources while maintaining accountability under CMS, MNDHS, and LSS guidelines.

You will promote enrollee knowledge, independence, and self-advocacy, requiring strong communication skills, attention to detail, and the ability to work

Qualifications

• Licensed Social Worker or Registered Nurse licensed to practice in Minnesota.
• Minimum of one year of experience with assessments and knowledge of community resources.
• Valid driver's license.
• Must pass background study and motor vehicle check.
• Strong written and verbal communication skills.
• Computer skills including Microsoft Office Suite.

Responsibilities

• Conduct initial and annual comprehensive assessments for enrollees.
• Create an individualized Plan of Care and coordinate services.
• Promote and enhance enrollee knowledge, resources, strengths, and independence.
• Ensure service accessibility with culturally appropriate, person-centered care.
• Maintain fiscal and professional accountability in compliance with rules and regulations.
